Used by HOA homeowners to submit requests for architectural and/or
landscape changes. As a courtesy,
completed form must be submitted to the ARC at least 5 days prior to
scheduled ARC meeting to be considered at the ARC meeting, unless appearing
in person. |

Used by an HOA member if the ARC, during the review of a homeowner’s
request, deems it valuable to obtain neighbor feedback. The ARC will request the input of affected
neighbors utilizing this form for Neighboring Property Review. |

Used by an HOA member to notify our management company of a homeowner
that may be in violation of our HOA Governing Documents. |

Used by an HOA member to notify our Board of Directors about tenants
renting their property per our HOA Governing Documents. |

Directors and Committee Chairs to submit HOA expense reimbursements. Receipts must be attached |
